About this Facility

West Lafayette Community Based Outpatient Clinic strives to be a patient-centered integrated health care organization for Veterans providing excellence in health care, research, and education. West Lafayette Community Based Outpatient Clinic provides outpatient services for mental health and substance abuse.

Since 1898, their buildings, facilities, patients, and missions have changed, but remaining constant is VA Illiana Health Care System’s endeavor in improving the health of the men and women who have so proudly served the nation. Being the 8th oldest VA facility, they consider it their privilege to serve patient’s health care needs in any way they can. Services are available to more than 150,000 veterans living in the surrounding 34-county areas of Illinois and Indiana.

VA Illiana Health Care System offers services to patients who served in the active military, naval or air service and are separated under any condition other than dishonorable. Current and former members of the Reserves or National Guard who were called to active duty (other than for training only) by a federal order and completed the full period for which they were called or ordered to active duty may be eligible for VA health care as well.

The VA Medical Center is on the Danville bus line; departures and arrivals are scheduled frequently throughout the day. The pick-up and drop-off is located in front of Building 98 at the main entrance of the medical center. Other pick up shelters are located in front of Building 58, 101, and 102.

For convenience and safety, they have special waiting areas throughout the facility. VA Illiana Health Care System has a Police Service. Their officers provide 24-hour patrols of the facility and parking lots. For general police assistance during normal business hours, please dial 45146. In case of an emergency, dial 45000.

The Suicide Prevention Program is to reduce the possibility of suicide through increased awareness and education, and increased access to clinical intervention and through consistent and supportive follow-up to the Veterans in Crisis. This resource provides confidential support to Veterans should they feel as though suicide is the only option. The hotline is available 7 days a week, 24 hours a day, 365 days a year. The coordinator is staffed by trained professionals to provide support and follow-up to those callers.

The Substance Abuse Rehabilitation Program (SARP) is an intensive outpatient program that runs Monday through Friday for six weeks. The Veterans participants while in the treatment usually commute from home, reside at the Salvation Army in Champaign, or reside in our Psychosocial Residential Rehabilitation Treatment Program (PRRTP). Travel assistance may be available to those commuting from home.

The LRC is responsible for coordinating the evaluation of recovery implementation at this facility and reporting results to the National Recovery Coordinator (NRC). With guidance from the NRC, the LRC will implement measures to assess recovery policy and procedure adherence, as well as assess the extent that the recovery model is embodied in the mental health programs.
