Threshold–West 47th St. is an outpatient mental and behavioral health care facility for adults in Chicago, Illinois. The center specializes in medication assisted treatment (MAT), intensive outpatient programs (IOP), general outpatient programs (OP), and aftercare programming.
Dedicated services are available to provide targeted treatment for various demographics. Specialized treatment is provided for adolescents, young adults, seniors, justice-involved persons, homeless and indigent persons, young mothers, persons with hearing impairments, military personnel and veterans, and persons with co-occurring addiction and mental illness.
The intensive outpatient program (IOP) ensures high-level supervision and support for clients in early recovery, those stepping down from inpatient care, and those at an elevated risk of relapse. Clients receive medical and mental health assessments and personalized care planning. Those in alcohol and/or opioid recovery may enroll in the evidence-based medication assisted treatment (MAT) program for longer-term pharmacotherapy. Clients engage in intensive individual, group, and family counseling drawing on proven modalities, including CBT and DBT. The program prioritizes recovery-focused life-skills training that addresses topics such as coping, self-care, and relapse prevention. Clients may choose from a variety of treatment approaches, including 12-Step or SMART Recovery.
Their general outpatient program (OP) and aftercare services ensure a complete continuum of care aligned with clients’ evolving needs. These services may include step-down support, 12-Step program facilitation, employment and housing assistance, and referrals for medical, mental health, and social service programs.
