Jan's Place, in Litchfield, Minnesota, is a licensed adult foster care (AFC) program for clients experiencing mental illness and/or addiction and requiring round-the-clock supervision. They offer short and long-term programming, crisis intervention and patient stabilization, and aftercare planning and support, including dedicated services for young adults, seniors, and persons with co-occurring addiction and mental illness.

The supportive housing program allows clients to focus on their recovery in a highly structured and therapeutic environment featuring comfortable, home-like amenities. They also engage in intensive individual, group, and family counseling and robust, recovery-focused life skills training addressing topics such as coping, wellness, disease and medication management, and relapse prevention. The program promotes clients’ successful reintegration into the home, workplace, and community through extensive independent living skills development, including job readiness, financial and household management, and self-care.

Their aftercare services ensure a complete continuum of care aligned with clients’ evolving needs and may include transitional support for clients stepping down to outpatient treatment, housing and employment assistance, and referrals for additional services.

Jan’s Place is state licensed and accepts private insurance, state and federal funding, and self-pay. Financial aid is available.
