Gracepoint Tampa is a behavioral healthcare center in Tampa, Florida. They provide comprehensive mental health programs, addiction services, and dual diagnosis treatment.

Resilience, Endurance, and Continued Hope (REACH) is Gracepoint’s outpatient program for individuals over the age of 18. The program treats addiction and/or other co-occurring conditions, using evidence based practices that include a clinical assessment, individualized treatment planning, individual and group therapy, relapse prevention techniques, skill building, coping skills, physical fitness, employment and job readiness, independent living, and recovery support groups. Sessions are held several days per week, depending upon the individual’s circumstances.

Care coordinators at Gracepoint assist individuals aged over 18 with transitions such as hospitalization or inpatient treatment, providing preventative care, arranging transportation, assistance with acquiring primary care or behavioral healthcare, making community referrals, and removing barriers to health and wellness.
