About this Facility

For Action in Togetherness Holdfast - FAITH is a Non-profit rehab located in Chicago, IL. For Action in Togetherness Holdfast - FAITH is dedicated to providing formerly incarcerated inmates from the State of Illinois Correctional facilities with services and referrals to active social service resource agencies that are currently available and operative within the city and suburban communities.

The intent of For Action in Togetherness Holdfast is to help these men and women with special needs services as they re-enter the community, reunite with their families, and begin to reconstruct their lives.

Faith Inc. provides men, women, and youth with many services to help them get back on their feet after being incarcerated. To learn more about additional resources, FAITH Inc. holds community advisory group meetings on the first Friday of every month. Community meetings are open to the general public. The meetings are a great way to receive helpful information and participate in the planning process to determine what is needed in the community. It is also a helpful way to receive up-to-date resources from community activists.

Treatment

Mental Health

Drug rehabs often provide treatment for people seeking help for both mental health and substance abuse. The programs are usually offered on an inpatient or outpatient basis. Each personalized treatment plan includes a number of different mental health therapies and services, like cognitive-behavioral therapy (CBT), experiential therapies (equine and animal therapy, outdoor therapy groups, and individual / group counseling sessions designed to simultaneously address both mental health and substance abuse. Standard treatment modalities include relapse prevention education and emotional coping skills training.

Level of Care

Outpatient

Clients in outpatient rehab receive a variety of services based on their unique and evolving needs. Typical outpatient treatment modalities include individual, group, and family counseling, recovery-focused life skills training, and community reintegration support. Many programs also provide medication assisted treatment (MAT) for clients recovering from alcohol and/or opioid dependency. Clients exiting inpatient treatment or those who are at an elevated risk of relapse may engage in more robust treatment, such as intensive outpatient (IOP) programming.

Aftercare Support

Completing a drug or alcohol rehab program shouldn't spell the end of substance abuse treatment. Aftercare involves making a sustainable plan for recovery, including ongoing support. This can include sober living arrangements like halfway houses, career counseling, and setting a patient up with community programs like Alcoholics Anonymous (AA) or Narcotics Anonymous (NA).

Family Therapy

Research clearly demonstrates that recovery is far more successful and sustainable when loved ones like family members participate in rehab and substance abuse treatment. Genetic factors may be at play when it comes to drug and alcohol addiction, as well as mental health issues. Family dynamics often play a critical role in addiction triggers, and if properly educated, family members can be a strong source of support when it comes to rehabilitation.

In individual therapy, a patient meets one-on-one with a trained psychologist or counselor. Therapy is a pivotal part of effective substance abuse treatment, as it often covers root causes of addiction, including challenges faced by the patient in their social, family, and work/school life.

Life skills trainings involve all the skills a person must have in order to function successfully in the world. These include time management, career guidance, money management, and effective communication. Truly successful addiction recovery is based on the ability to not only live substance-free, but to thrive. Life skills teaches the practical necessities of functioning in society, which sets clients up for success in life, and therefore sobriety.
