Cityteam – House of Grace is a social service organization in San Jose, California that offers drug and alcohol rehab for adults and youth with substance use disorders. They also provide crucial support services such as referrals, mental health services, transitional living, and restorative programs.
Cityteam – House of Grace uses an interdisciplinary, community-based approach to treatment for individuals with substance use disorders and co-occurring mental health diagnoses. Using evidence-based practices, such as Assertive Community Treatment (ACT), staff provide counseling, skills training, peer support, and other social services. The IOP is offered several days per week and is open to adults over 25 and youth aged 18 to 24.
The residential program is for individuals with substance use disorders who are experiencing homelessness or are at risk of losing their home. Participants in this program receive counseling, skills training, peer support, and other social services. Residential support is available to adults over 25 and youth aged 18 to 24.
The peer support program is provided by individuals who have first-hand experience with addiction and recovery. Each peer goes through a training program which requires them to be certified by the state. Peers can support individuals as they make a recovery plan, access services, attend meetings, and work toward achieving their goals.
