While in short term residential care, clients will receive individualized aftercare plans, including referrals and transitional support as they move into longer term residential care or other recovery programs. Aftercare support may also include referrals and transitions to halfway houses or sober living facilities. Clients transitioning to outpatient care may receive peer coaching and support in accessing community based 12 step and 12 step alternative programs. Referrals for mental and physical healthcare, employment, transportation and housing assistance, and other social services may also be provided based on the client’s individual needs and goals.
